Welcome to the Internet Privacy & Security Statement (“Privacy & Security Statement”) for the Prostate Cancer Awareness Alliance of Northeast Pennsylvania (hereinafter defined as ‘PCAANEPA’). The Internet Site for the PCAANEPA is hosted by Blue Cross of Northeastern Pennsylvania (hereinafter defined as “BCNEPA”), a proud sponsor of the PCAANEPA. The terms referenced herein throughout this privacy & security statement are specific to BCNEPA, the hosting facility of the PCAANEPA website.
The purpose of this Privacy & Security Statement is to summarize and disclose all information, collecting and disseminating practices of BCNEPA concerning the web site for the PCAANEPA, www.pcaanepa.org (“Public Pages”).
This Privacy Statement is effective as of May 19, 2009 and was last updated on May 19, 2009. However, both PCAANEPA and BCNEPA reserves the right to change this Privacy & Security Statement as necessary and without notice to users of this site. Collection of Personal Information

BCNEPA will not be tracking personal data. The only data recorded will be the volume of visits as tracked by IP address and pages accessed.
In addition, utilization statistics for all traffic on our public pages is being collected by a third party.

Use of Data Collected from Input Forms

The current site does have a limited ability to input personal information onto data input forms for contact information purposes only. Data entered consists of first name, last name, email address and an area to document or submit a question for review. The information collected on the input forms is distributed to an email box of the PCAANEPA, however, it is not stored in a database. Use of this information is solely for responding via email to inquiries submitted through the Contact form. Prior to entering such data, users should carefully review this statement and BCNEPA’s description of its limited security features.

BCNEPA will not sell, trade or disclose data to non-affiliated third parties for any purpose.

Logging and Tracking
BCNEPA will not be logging or tracking personal data. BCNEPA will be collecting certain activity including, but not limited to, successful and failed log-in attempts as well as usage statistics for all functional components. The only other data recorded will be the volume of visits as tracked by IP address and pages accessed.

Privacy/Security Breaches
BCNEPA is concerned about the privacy and security of this site and its visitors. Because the privacy and security of this site cannot be fully protected from breaches, BCNEPA has established an address for visitors to report breaches and suspected breaches of the privacy and security of this site.
